The Seminole County Community Health Division’s Healthy Lifestyles program works to help vulnerable populations with managing their chronic health conditions and address other determinants of health. Through a combination of home visits and outreach events, Case Managers, who are also Certified Community Health Workers, seek to assist clients with resources out in the field. Clients can receive items such as tools to aid in medication management, nutritional education, and referrals to partner agencies for further assistance.
Certified Community Health Worker, Lidia Galeas, has been with Seminole County since May 2023 and holds a dedicated background in Public Health. She is a committed advocate for advancing health education, outreach, and fostering relations to reduce health disparities.
Galeas earned her Bachelor of Science in Biology from Seminole State College and has previously worked as an Outreach Coordinator for Hispanic Health Initiatives in DeBary and as a Medical Assistant for The Women’s Center in Orlando.
She continues to work toward enhancing health opportunities for others through her service in our local community, including outreach events and home visits.
When not working to improve healthcare across our region, she enjoys spending time with her loved ones, her dog Danny, enjoying self-care routines, and cooking new recipes.
Certified Community Health Worker, Chantel Reed, has been with the Community Health Division since 2023 and works diligently to bridge healthcare gaps across the County through outreach and meaningful relationships.
Prior to this role, Reed worked as a Psychiatric Technician at AdventHealth in Orlando where she assisted at-risk clients with appropriate interventions and precautionary measures. She earned her Bachelor of Science in Psychology with a focus on Neuroscience from the University of Central Florida.
When not focused on improving the health conditions of clients and addressing social determinants of health, Chantel spends her spare time working on various arts and crafts projects, reading, and playing volleyball.
